This presentation will discuss the importance of making community connections for individuals with substance use disorders re-entering the community. Drs. From Allegheny Health Networks will discuss the connection between the RIvER clinic and Allegheny County Corrections, how the connection was established, how they work toward a smooth transition of care, and the outcomes they have seen as a result of this collaboration.
